Definition
A fixed dollar amount you pay for a covered health care service, regardless of the total cost. For example, a $30 copay means you pay $30 every time you see a primary care physician, while the insurer covers the rest. Copays apply per visit or per prescription and typically do not count toward your deductible (though they do count toward your out-of-pocket maximum on most plans).
